Privacy statement
In accordance with the College of Occupational Therapists of Ontario (COTO) and Personal Health Information Protection Act (PHIPA), Ruby Knafo OT Reg. (Ont.) will maintain documentation of services. Your personal health information will be kept confidential, will not be disclosed electronically or otherwise without your express permission unless required by law. Under the Personal Health Information Protection Act, 2004 (PHIPA), Occupational Therapists are permitted to disclose personal health information about an individual if they have reasonable grounds to believe disclosure is necessary to eliminate or reduce significant risk of serious bodily harm to a person or group of persons. This disclosure is sometimes referred to as a duty to warn. Although duty to warn is not a legislative term, it is a provision in the law. If the risk is sufficient, then the duty to warn serves as the legal authority to release the information.
Only information that is reasonably necessary to provide service will be collected. Paper records are kept in a locked cabinet, and electronic information is stored a on password protected personal device. You are entitled to view your records at any time by submitting a request to ruby@unionyogastudio.ca .
